Transplant Coordinator-Kidney Transplant

New Jersey transplant program is seeking a Pre-Kidney Transplant Coordinator.  The position provides support and education to patients throughout the pre-transplant process. Oversees and/or coordinates the clinical, psychosocial and financial needs of the transplant patient. Coordinates pre-operative tests and consults to determine transplant candidacy and maintains the program’s waiting list which includes providing regular follow up on patient status while waiting for transplant. Facilitates transplant by organizing patient and transplant team. Monitors inpatient status, discharge needs and follow-up care per program standards. Documents and maintains patient transplant information in the electronic medical record to ensure regulatory compliance. This position maintains all policies and procedures, and works collaboratively with the nurse manager, Physicians, social worker, financial coordinator, and support staff to provide comprehensive care to the recipient.

Qualifications:

1. Graduate of a NLN/AACN accredited program in nursing.
2. Bachelor’s degree in nursing.
3. Minimum of 5 years in acute care medical/surgical setting. Transplant experience preferred.  CCTC is required within 30 months of hire.
